Patient Policies

  • Payment is required at the beginning of your time of service. A credit or debit card must be placed on file when booking your appointment. Accepted payment methods at the start of your physical therapy session include credit cards, HSA/FSA cards, checks, and cash. Receipts are available upon request and can be submitted for out-of-network reimbursement if applicable.

  • Refunds will be provided only in the following circumstances:

    • Billing errors.

    • Unused session packages canceled within 30 days of purchase. Refunds for package sessions will be pro-rated based on the standard per-session rate.

    • Overpayments. All refund requests must be submitted in writing.

  • If you arrive late for your session, we will accommodate you to the best of our ability within the remaining time. Sessions will still end at the scheduled time to respect the next patient’s appointment. If you are more than 15 minutes late, the appointment may need to be rescheduled and subject to the cancellation fee.

  • All patient information is treated as confidential and handled in compliance with HIPAA regulations. Records are securely stored, and any electronic communication is conducted using encrypted systems to protect your privacy. No paper records are kept on site, all patient information is stored using Prompt’s electronic medical record.

  • Consistently missing or canceling appointments disrupts progress for you and others. Three or more missed or canceled appointments without proper notice may result in the inability to book future sessions. Refer to the cancellation policy for more details.

  • The Equipt for Life clinic is a space of mutual respect. Disrespectful or inappropriate behavior toward staff or other patients is not tolerated and may result in immediate discharge from care.

  • When you’ve achieved your treatment goals or no longer require active care, We will provide a discharge summary outlining progress and recommendations for ongoing self-management. You are always welcome to return for follow-up care as needed.

Clinical Protocols

  • All patient information is treated as confidential and handled in compliance with HIPAA regulations.   • Patients are required to complete intake forms, including a medical history and consent for treatment, 12 hours before their first appointment. All treatments and assessments will be documented securely and updated after each session to ensure continuity of care.

  • All treatments are tailored to your unique goals and needs, with a focus on evidence-based care. Each session includes one-on-one time with a Doctor of Physical Therapy - no PT assistants or PT techs - ensuring the highest level of professional attention. will be involved in your care.

  • To maintain a clean and safe environment:

    • Equipment and surfaces are sanitized between each patient.

    • Hand sanitizer is provided and encouraged.

    • We follow all CDC guidelines for infection prevention.

  • Appointments can be scheduled or rescheduled online or by phone. To secure your preferred time slot, book at least one week in advance. Short-notice cancellations will be subject to the cancellation policy.

  • In case of an emergency during a session, appropriate first aid will be administered, and emergency services will be contacted if necessary. You will be referred to your primary care provider or another medical professional if your condition requires further care.

  • The clinic is open Monday through Friday, 6 AM - 10 AM & 3 PM - 6 PM. Admin are available 6 AM - 6 PM to answer any questions, receive bookings, or discuss changes in plans of care. Inquiries received after 6 PM will be returned the next business day.

  • One (1) accompanying child or family member is welcome in the clinic during your appointment. For safety and to ensure a focused session, children must be supervised at all times. Family members may be present in the treatment area upon prior approval.

Ethical Policies

  • To support patients with financial challenges, we set aside 20% of each session cost to provide discounted rates for individuals at or below 200% of the federal poverty line. Applications for reduced rates must include proof of income and are subject to approval.

  • As a licensed Doctor of Physical Therapy, your DPT will provide care within the scope of their training, including movement assessments, manual therapy, and exercise prescription. If a need is identified for care beyond their scope, you will be referred to an appropriate provider.

  • Care may be discontinued if:

    • Treatment goals have been achieved.

    • Non-compliance with the treatment plan inhibits progress.

    • Safety concerns arise.

    • Disrespectful or inappropriate behavior occurs. Patients will receive a referral to another provider if needed.
